Registration Information

Status- We are currently taking on new pattients. Please see information below.

Practice Area

Giggs Hill Surgery serves the area of Thames Ditton, Long Ditton, Weston Green, Hinchley Wood and parts of Esher, East Molesey, Surbiton. See Practice area Map.

Permanent Registration

People who have moved into the area (see map) intending to stay longer than 3 months, and are currently registered with another NHS doctor, should apply on a permanent basis and will be registered as NHS patients. See How to Register below.

How to register

Please call in to the Surgery and tell the Receptionist you wish to register . You will be given a number of forms to complete (if you have not done so already). Our Receptionist will be very happy to help you complete the forms. (Sorry. We are unable to offer on-line registration).

You will be provided with our Practice Booklet

Details required to complete the registration process. Best of all have your current Medical Card

Name; Address; Post Code; Date of birth; NHS number (the new 10 digit one); Place of birth (or borough if London); Previous Address; Previous GP;
If from abroad; Date entered the UK & Your first address in the UK; If returning to UK the date of return and last address.

Application to the doctors list can be refused in rare circumstances.

On satisfactory completion of the registration form and if your application is accepted the details will be entered on our computer. The registration details are sent electronically to the Primary Care Support Service. They will officially confirm registration and request the patient's notes from your previous GP. Registration is immediate and you can see a doctor without having to wait for approval by the PCSS (subject to proof of NHS entitlement).

New Baby Registrations
New baby registrations are slightly different. When you go to the Registrar to register the birth of your baby he/she will give you an FP58 Application to Join a GP Practice and Birth Certificate. We will need sight of these documents to register your child with the practice.

Registration of Patients from Overseas
If you are coming to the UK and staying you are entitled to NHS care after you have been a resident for 6 months. You will need to register in the same way as detailed in the section above headed “Permanent Registrations”.

After Registration

Once your registration is confirmed you will be contacted by our Health Care Assistant, who will make an initial appointment for a New Patient Check. Please arrive in ample time in case we require any further information. You should bring your completed new patient questionnaire (if not already returned to us) and a urine sample. Sample bottles are available from reception. Permanent registration may be dependent upon attendance at this check up.

We operate a shared list system whereby any patient can see any doctor. We do suggest that it is preferable to stick with one doctor for continuing care of a specific medical problem. You should not wait until an emergency arises to register as this may delay you getting an appointment and you should include all members of family when registering.

New patients will be Registered with either Dr Matthews, Dr Keating or Dr Crow. Exceptions are the new babies of existing patients, who will be registered with the doctor of the family. We operate a shared list system whereby any patient can see any doctor.

With the advent of terrorism and health tourism the government are shifting the onus on ensuring that people registering for healthcare are not only entitled to NHS care but who they say they are. In the past this role has been undertaken by the PCSS (Primary Care Support Service) who have taken on trust the registrations we have forwarded to them. With the advent of the NHS CRS (Care Records Service) this is all the more necessary as we will begin to share information around the NHS family. Most patient's first port of call is their doctor and as such it is the surgery they go to register or change their address.

Entitlement to NHS care is based on you having been a UK resident for a continuous period of 6 months prior to registration. It is not based on the payment of National Insurance Contributions or Tax.

With immediate effect the following documents will need to be supplied when registering with this practice. If you have any concerns please speak to one of our receptionists.

Supporting Documents Required

Photo Identification:
This should something official with your picture on like a driving license, student card, bus pass or work pass.

Legal document supporting your name:
a birth certificate or marriage certificate showing your birth name.

Confirmation of UK Residency
A recent bank statement, household bill or piece of junk mail showing your current UK address (one dated 6 months ago for those not previously registered with a UK doctor).

Change of Address:
One of the following: Bank Statement showing new address Household bill showing new address.

Removal from our list

The doctors have the right to remove any patient from the list if required. This is reserved for exceptional situations for example is cases of abuse, violence or other times where the doctor-patient relationship has irrevocably broken down. More commonly it is because we simply get notice that a patient has moved out of our area.
