Temporary Registration

Giggs Hill Surgery serves the area of Thames Ditton, Long Ditton, Weston Green, Hinchley Wood and parts of Esher, East Molesey, Surbiton. See Practice area Map.

Temporary Residents

If someone is unfortunately taken ill away from their usual place of residence, the NHS allows those patients to register as a Temporary Resident with a GP in the area they are visiting. This can be for a period of up to 3 months. There are two catagories - up to 2 weeks and 2 weeks to 3 months. A Temporary Registration Form will need to be completed . You will need to supply the full details of the GP you are registered with at your usual place of residence. An NHS medical card will make the temporary registration process easier and is proof of entitlement to NHS treatment.

For those who are not normally resident in the UK, or who have been living in the UK for less than 3 months special rules apply. For residents of EU and some other countries, reciprocal agreements apply. On production of an valid E111 (EU) they can be seen for treatment in an acute illness or emergency. Most of the cost of treatmant can then be claimed back from their "home" health department. The E111 does not cover ongoing care of medical conditions. A British passport does not entitle a person normally resident outside the UK to health care under the NHS (being resident in the UK for more than 6 months does).

We will provide our services on a private basis to those without proof of NHS entitlement. A list of fees is available at reception.

Details required to complete the temporary registration process. Best of all have your current Medical Card

Name; Date of birth; NHS number (the new 10 digit one); Temporary Address and Telephone number within our Practice area (see map); Home (permanent) Address & Post Code; Name and Address of usual GP.

Please arrive in ample time before your appointment to enable completion of the paperwork. (view a sample of the Temporary registration form)

Temporary Residents Up to 2 Weeks
This type of registration is for any patients visiting the area for 2 weeks or less and are currently registered with a GP surgery in the UK. You will need to complete a temporary registration form (Blue) giving details of your current doctors name and address, along with the address you normally reside at and the address you are staying at.

Temporary Residents 2 Weeks to 3 Months
This type of registration is for any patients staying in the area for a short period of time from 2 weeks to 3 months and are currently registered with a GP surgery in the UK. You will need to complete a temporary registration form (Blue) giving details of your current doctors name and address, along with the address you normally reside at and the address you are staying at.

In addition we may also require proof of identity.

If you are staying in the area longer than 3 months you will need to register permanently.

Immediately Necessary Patients

If you visiting the area for 24 hours or less and are suffering from a condition that needs immediate attention we will see you as an “Immediately Necessary Patient”. You will need to complete a “Blue” Registration form and have details of your doctor’s name and address. The registration then follows the same path as a Temporary resident of less than two weeks, as above. This registration covers you only for the day on which you receive the “Immediately Necessary Treatment”. You will be asked to provide some form of identification.

Private Patients

We will provide our services on a private basis to those without proof of NHS entitlement. A list of fees is available at reception. Acute medical conditions will be seen by the "Doctor of the day". Please contact our Receptionist for guidance if you are in doubt about your entitlement to NHS registration. If you have arrived from abroad, and wish to register, you will be treated as a Private patient for 6 months ( to satisfy NHS residency conditions). Application to the doctors list can be refused.
